Waiver, Release, and Assumption of Risk Form

This form is an important legal document. It explains the risks you are assuming by participating in the . It is important that you read and understand it completely. After you have done so, please indicate your agreement to this Waiver, Release and Assumption of Risk From by printing your name legibly in the two paragraphs below and sign in the spaces provided at the bottom of this form.

Waiver, Informed Consent, and Covenant Not to Sue

I have volunteered to participate in a competition under the direction of & OPSM which will include, but may not be limited to, high intensity training and competition. In consideration of the OPSM event agreement to instruct, assist, and train me, I do here and forever release and discharge and hereby hold harmless OPSM event and their respective coaches, heirs, assigns, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in this or any exercise program put on by OPSM Event including any injuries resulting therefrom. THIS WAIVER AND RELEASE OF LIABILITY INCLUDES, WITHOUT LIMITATION, INJURIES WHICH MAY OCCUR AS A RESULT OF: (1) EQUIPMENT THAT MAY MALFUNCTION OR BREAK; (2) ANY SLIP, FALL, OR DROPPING OF EQUIPMENT; AND (3) NEGLIGENT INSTRUCTION OR SUPERVISION.

Assumption of Risk

I recognize that the might be difficult and strenuous and that there are dangers inherent in competing in the . I acknowledge that the possibility of unusual physical changes during exercise exists. These changes include abnormal blood pressure; fainting; disorders in heartbeat; heart attack; and, in rare instances, death. I understand that as a result of my participation, I could suffer an injury or physical disorder that could result in my becoming partially or totally disabled and incapable of performing any gainful employment or having a normal social life. I recognize that an examination by my physician must be obtained prior to involvement in this . I acknowledge and agree that I assume the risks associated with any and all activities and/or exercises in which I participate.

I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A FULL RELEASE OF ANY LIABILITY OPSM MIGHT OTHERWISE INCUR AS A RESULT OF MY PARTICIPATION IN THE . BY SIGNING THIS DOCUMENT, I AM WAIVING ANY RIGHT I OR MY SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST or OTHERS REFERRED TO IN THIS DOCUMENT FOR ANY NEGLIGENCE INCLUDING NEGLIGENCE OF ITS EMPLOYEES, AGENTS, OR CONTRACTORS.

COVID PROOF OF VACCINATION (CURRENTLY NOT APPLICABLE)*

Due to Facility Covid Restrictions, all participants are required to provide a proof of Covid-19 vaccination via QR code at the OPSM event. Proof of Covid Vaccination must be a minimum of double dose. Check-in will be at the entrance of the event Facility, so please ensure you have a piece of ID as well. If you do not have proof of vaccination, then participants are required to show a doctor's exemption note.

*Note that this does not apply to OPSM events within America. Therefore, particpants attending any event within USA, can provide COVID-19 Negative test result (2 weeks in advance) or COVID-19 Vaccine Proof (paper or digital copies) to be enable to participate at the hosted event.

Photography and Audio/Video Recording

Acknowledgement

I hereby give OPSM permission to videotape, photograph, and record my image and or likeness. I understand that such taping or recording may be used at the sole discretion of OPSM. I also understand by giving my permission, I am in no way endorsing OPSM or any product(s) distributed by OPSM.
